Who can receive home health care?

Persons with acute illnesses or conditions that require skilled services of a nurse and/or therapist qualify for home health care. Home health care is prescribed by a physician.

How is home health care arranged?

Generally, hospital case managers or social workers arrange for home health care for individuals being discharged from the hospital. In some cases, the physician can directly arrange for home health care. Clients or their loved ones can also call our office at (703) 379-1008 and arrange for home care if they need our services.

How do I get started?

Please call us at (703) 379-1008 to learn more or set up an appointment for a complimentary consultation with a registered nurse.

Who is eligible for home health care?

Diamond Home Health Care, Inc. provides skilled and personal care services to adults of all ages.

What are the skilled services that you provide?

The skilled services we provide include: Skilled Nursing Care (LPN, RN), Physical Therapy, Speech Therapy, Occupational Therapy, Medical Social Services and IV Therapy.

What are the personal care services that you provide?

The personal care services we provide include: Certified Home Health Aide, Personal Care Attendant, Companionship, and Private Duty.

Can I always get the same caregiver or the caregiver of my choice or my family’s?

Diamond Home Health Care, Inc. strives to offer the most continuity in the care it provides. We will make every effort to provide the same caregiver or the care giver of your choice as much of the time as possible.

How are caregivers selected?

Our caregivers are selected based on verification of certification/licensure, review of employment history and references and personal interviews. In addition, all of our employees must pass criminal background checks and drug tests.

How do I reach your office after business hours?

Our home care agency office staff is available 24 hours a day, 7 days a week and 365 days per year. An on-call person is available to help you after regular business hours.

Will I get service immediately after I contact your office?

Usually, we conduct a needs assessment first before providing services. We need to coordinate with your physician to design an appropriate care plan. We encourage you to contact us and discuss your care requirements and options because the services we provide to you will be tailored to your specific needs.
